This is a role in a Catholic home hospice whose mission is to provide support, education and care that honors the dignity of each person served. In this role the Dirctor of Clinical Services serves as a supervisor to all field staff to assure adherence to agency policies and federal and state regulations; serves as liaison between clinical staff and operations staff; and as liaison among the Agency, facilities, the medical and general community. 

Volunteer Program Manager

In this role the Volunteer Program Manager is responsible for the management and coordination of volunteer services required per individualized Interdisciplinary Team (IDT) care plans.

Hospice Registered Nurse (RN)
Colorado

This is a role in a Catholic home hospice whose mission is to provide support, education and care that honors the dignity of each person served. In this role RNs are responsible for the provision of nursing care to hospice patients in coordination with the interdisciplinary team (IDT) that is consistent with the goals, definitions, philosophy, and mission of the Hospice. 

Hospice Social Worker
Colorado

This is a role in a Catholic home hospice whose mission is to provide support, education and care that honors the dignity of each person served. You will be responsible for the provision, direction, supervision, and evaluation of medical social services to patients in the hospice environment.

Hospice Certified Nursing Assistant (CNA)
Colorado

This is a role in a Catholic home hospice whose mission is to provide support, education and care that honors the dignity of each person served. CNAs will provide skilled personal care and comfort measures to clients in the home setting by performing the following duties under the supervision of a RN.
